Shaklee’s Workplace Violence Prevention Plan (WVPP) is intended to identify, evaluate and correct workplace violence hazards; prevent and mitigate workplace violence incidents; and otherwise address workplace violence matters in accordance with California Labor Code section 6401.9.

Forms:

  • Workplace Violence Hazard Identification Form
    Use this form to identify potential workplace violence hazards specific to your work area, operation or department. Additionally, identify potential corrective measures that the company should consider in addressing the potential hazard. Provide as much detail as possible. Use additional pages if necessary. Submit this form to Human Resources or the WVPP administrator(s) or other individuals designated to receive such information under the company’s WVPP.

 

  • Workplace Violence Hazard Inspection Form
    Use this form to help identify, evaluate and document potential hazards for correction in accordance with the company’s workplace violence prevention plan (WVPP). This is a general inspection form, some provisions of which may not be applicable to the specific worksite being inspected. Fill out all relevant portions of this form with additional notes and comments as needed. Give completed form to the WVPP administrator or other individual(s) designated to receive such information under the company’s WVPP.

 

  • Workplace Violence Incident Reporting Form
    Following the event of a workplace violence incident, to help the company investigate and respond appropriately, please complete this form describing the circumstances of the incident and return it to the appropriate supervisor, HR personnel or other individual(s) designated to receive such information under the company’s Workplace Violence Prevention Plan. Please be as specific as possible in order to help the company gain an accurate understanding of the incident in order to respond appropriately as part of its ongoing efforts to correct and prevent workplace violence hazards and maintain a safe working environment.

 

  • Workplace Violence Incident Log
    Use this form to log workplace violence incidents.  The completed log form should be under the custody of the WVPP Administrator.
